Pornography-induced erectile dysfunction (PIED) refers to a pattern of conditioned arousal difficulty with real-world partners following extensive pornography consumption, the proposed mechanism involves dopamine desensitization in the reward pathways of the nucleus accumbens, creating a mismatch between the high novelty stimulus of pornography and real-world sexual encounters, but the evidence base for this as a discrete physiological syndrome is Early (3), with significant controversy about prevalence and mechanism (Park et al., Behav Sci, 2016).

The practical challenge for physicians is that psychogenic erectile dysfunction and vascular erectile dysfunction are difficult to distinguish without nocturnal penile tumescence testing, and many men in their 30s–40s who present with ED have both components operating simultaneously. The cardiologist's position: rule out vascular disease first (because it is dangerous and concrete), then address psychological and behavioral contributors. Self-reported PIED is common in online forums and may represent genuine conditioned arousal pattern or may be a mislabeling of vascular ED. Either way, a cardiovascular assessment is warranted before assuming the etiology is behavioral.

What to do: Do not assume your erectile dysfunction is pornography-related without having vascular disease ruled out. Get the cardiovascular workup first.
