What is tirzepatide and how does it compare to semaglutide for the heart?
Tirzepatide (Mounjaro for type 2 diabetes, Zepbound for weight management) is a dual GIP/GLP-1 receptor agonist, it activates both the GLP-1 receptor and the glucose-dependent insulinotropic polypeptide (GIP) receptor simultaneously. The dual activation produces larger weight loss than semaglutide in direct comparisons: the SURMOUNT-1 trial showed approximately 15–22% body weight reduction with tirzepatide versus 10–15% for semaglutide.
The cardiovascular outcomes data for tirzepatide is not yet complete. The SURMOUNT-MMO cardiovascular outcomes trial is ongoing. Surrogate biomarker data is very promising, tirzepatide produces greater reductions in blood pressure, hs-CRP, ApoB, and insulin resistance compared to semaglutide, but the definitive cardiovascular event data that SELECT provided for semaglutide does not yet exist for tirzepatide. In a patient choosing between these agents for cardiovascular risk reduction specifically, semaglutide has the stronger current evidence base. For combined weight loss and metabolic optimization, tirzepatide's superior weight loss profile may be preferred, with the understanding that cardiovascular outcome trial confirmation is still pending.
