How does semaglutide reduce cardiovascular events — is it just the weight loss?

Weight loss from semaglutide contributes meaningfully to the cardiovascular benefit, but it does not explain the entire effect. In the SELECT trial, participants lost a mean of 9.4% of body weight compared to 0.9% on placebo, and that fat loss, predominantly visceral adipose tissue, reduces the pro-inflammatory cytokine production that drives hs-CRP elevation, improves insulin sensitivity, lowers blood pressure, and reduces sleep apnea severity. Each of these changes independently reduces cardiovascular event risk.

What suggests the benefit is not entirely weight-loss-mediated: the cardiovascular event curves in SELECT began separating earlier in the trial than the weight loss curves could fully explain. The drug appeared to be reducing cardiovascular risk before substantial weight loss had accrued. This early curve separation is the hallmark of a drug with direct vascular biology. GLP-1 receptors in endothelial cells, when activated, improve nitric oxide bioavailability and reduce vascular inflammation directly. GLP-1 receptor activation in cardiomyocytes improves cardiac efficiency and reduces oxidative stress. These are weight-loss-independent effects that operate through the same receptor the weight-loss mechanism uses. (Lincoff et al., NEJM, 2023)

What to do: This mechanistic nuance matters for one practical reason: men who have modest weight loss on semaglutide may still be receiving meaningful cardiovascular benefit through the direct vascular pathways. Do not judge clinical success solely by the scale.
